Accessing your health information is easier than ever with FollowMyHealth. See a quick health summary, send messages, make appointments, pay a bill, and so much more.

  1. Make sure we have your correct email address to send you an invitation.
  2. Click the registration link in the email you receive from moc.htlaehymwollofnull@ylperon.
  3. Create your portal login using a unique username and Password
  4. Enter an invitation code provided by us (patient year of birth) and click “Agree” on the release of information.

Download the FollowMyHealth app. Search “FollowMyHealth” and log in with your username and password.
