Billing
Insurance does not pay for a sports physical. Wellness exams are allowed by most insurance companies every 12 months. If child is eligible, we can perform a wellness exam, fill out the sports/school physical forms, and bill insurance if you wish. Wellness visits are much more detailed and require different documentation, so please let us know your preference prior to the exam. Please check with your insurance plan to ensure wellness visits are covered.
